Important Forms
All forms must be filled in, signed, and turned in before the first day your child attends.
Please note that a medical form is only valid for one year. Your child can not attend school until the medical forms are updated and signed .

CHILD MEDICALS: A Health Care Practitioner must examine each child yearly, per state law. Each child must have a valid signed medical form and a list of their immunizations on file. All children must have the immunizations listed in the Ohio Revised Code, unless they are medically contraindicated or not medically appropriate for the child’s age.

If you are interested in sign up your child for the 2025-2026 please fill out the Registration Form below and either mail or drop with payment included to secure your spot.
Registration for classes are a first come first served basis. You will be notified if there is no room in the classroom you have requested and offered to be put on a waitlist.

To help prepare your child for the first day of school there are supplies needed. Below you will find a link for the current school year supply list.
Please have the items pre labeled with your child's name on them.

Families have a few options when it comes to paying tuition. Cash and check are accepted.

You can also set up for auto withdrawal, fees will apply.
Through Brightwheel you can set up payments. You would go into your child's Brightwheel account to set this up. There fee structure is:
Pass card processing fees to payers
Card Fee: 2.95%
ON
Pass ACH processing fees to payers
ACH Fee: 0.6%, $0.25 min, $2 max
Or
To use Vanco, there will be a one time charge of $15 to cover those service fees for the year.

This form is used to clarify if you would like to share your information with other families in your class: name, address, email, and cell. This is helpful to other families who would like to arrange playdates/parties and so on. Along with your child's picture being used.
This form also contains information on who my your child be released to.

This form is giving permission for your child to join their class in walking adventures. Such as walking around outside in the church's yard or visiting other areas of the building when special events occur.

Every summer the families who are currently enrolled for the next school year receive a letter from the school with important information.
